ACPS Facilities COVID-19 Screening for Employees & Visitors
Employees: All employees are required to perform a self-assessment for symptoms of COVID-19 before reporting to work and again upon building entry.
Visitors: All visitors to ACPS facilities will be required to undergo a health screening for COVID-19 symptoms upon building entry.
In the past 48 hours, have you experienced any of the following symptoms:
- fever (100°F or higher) or chills
- shortness of breath or difficulty breathing
- muscle or body aches
- new loss of taste or smell
- sore throat
- congestion or runny nose
- nausea or vomiting
In the past 14 days, have you been in close contact (within 6 feet for a total of 15 minutes or more over a 24-hour period) with someone with suspected or confirmed COVID-19?
In the past 10 days, have you been diagnosed with or presumed positive for COVID-19?
Are you isolating or quarantining because you may have been exposed to a person with COVID-19 or are worried that you may be sick with COVID-19?
Are you currently waiting on the results of a COVID-19 test?
Did you answer NO to ALL QUESTIONS?
