All students and employees should use this Daily Health Screening tool to decide if they should attend school or report to work. Please stay home if you are sick.

All students and employees should use this screening tool to decide if they should attend school or report to work. Please stay home if you are sick.

If you test positive for COVID-19, please stay home and contact your school nurse or supervisor as soon as possible.

Please note: Persons who have had recent confirmed or suspected exposure to an infected person are recommended to wear a mask for 10 days around others when indoors and to seek testing 5 days after exposure (or sooner, if they are symptomatic).

Question 1

In the last 5 days, have you tested positive for COVID-19?

YES — Please stay home and contact your school nurse or your supervisor as soon as possible.
NO — Proceed to Question 2.

Question 2

In the last 10 days, have you been exposed to someone who has COVID-19?

YES — Proceed to Question 3-A.
NO — Skip to Question 3-B.

Question 3-A

Do you have any COVID-19 symptoms?

YES — Please stay home.
NO — You may attend school or report to work.

Question 3-B

Are you experiencing fever, vomiting, diarrhea, shortness of breath, cough, sore throat, or sudden loss of taste or smell?

YES — Please stay home.
NO — You may attend school or report to work.
